I certify that I am 18 years of age or older and that the information on my
application is true and correct to the best of my knowledge. I understand that
La Garou is a fraternal organization founded to provide social, service and
educational opportunities for transmasculine people and their supporters. I
understand that Alpha membership is reserved for transmasculine people. I declare
that my application for membership has no hidden agenda. If selected as a member
of La Garou, I will uphold the ideals espoused by the club. I understand that
if my conduct be deemed inconsistent with the ideals and purpose of La Garou,
the club has the right to revoke my membership at its discretion; I also have
the right to withdraw from my association with La Garou at any time. I agree
to comply with and uphold the Bylaws, Rules and Traditions established by La
Garou. I understand that La Garou is not responsible for any transactions between
myself and any people or organizations I may contact through the club or its
events, nor may the club or its membership be held responsible in any way for
my statements or actions. By my signature on this application, I affirm the
above and agree to abide by the requirements, policies and procedures as established
for membership in La Fraternité du Loup-Garou.

If you are applying for Beta(associate) membership then a check or money order
for $25.00 must me paid to our Paypal account or sent to us in the form of a
check or money order. If you are applying to be accepted as a Cub(pledge) to
become an Alpha(full) member then please pay us $15.00 through Paypal, or send
us a check or money order with the application. In the event that membership
is not extended to the applicant, this fee shall be returned. Please make checks
or money orders payable to: La Fraternité du Loup-Garou. If paying by
